Please send your completed application (and any requested supporting documentation) to the AfS office (address below). Your application will then be reviewed and we will contact you. Priority for non-medical volunteers will be given to mission-funding entities.

Special instructions for filling out the Chinese visa application:

  • In Section 2.1 check the Other box, and write in: "Medical Mission"
  • Do not fill out sections 4.7, 4.8, 4.9, and 4.10. We will fill those sections out in the office.
